1. Fire Prep Topics of Interest – Blood Pressure Increase During Physical

I had an interesting question come across my desk the other day. I thought it might help more people who might be in the same situation.

Question:

I have a physical coming up soon and wanted to find out if anyone can help. Whenever I take my blood pressure at home it is around 130/70. Anytime a doctor or nurse takes it, it comes up to about 160/90. I've heard about the white coat syndrome, but how do I get around it during a medical exam? Any help would be appreciated.

Answer:

Yeah, it sounds like you have white coat syndrome, alright. Neither one of those numbers is perfect, yet I feel it could be dealt with very easily if you are willing to make some sacrifices. Make some lifestyle changes right now. You could do all of these things at once, or space them out for the 2 weeks before you go in for your physical.

For 2 weeks before:

No caffeine or alcohol. Either will bring mine up: from 100/60 to 120/80... that's a huge difference.... though not dangerous even at the higher end.

For 1 week before:

The above plus:

No salt (eat whole foods- no processed food. No TV dinners, no restaurant food.) Do some cardio each day: Easy to Moderate, 30 minutes, stretch afterwards.

During this week, if it were me, I would go get a good, relaxing massage. That might be overboard for some people. But the tension created from muscle spasm and little aches and pains we don't know we have can contribute to stress. Sometimes, if I work on someone who has a blood pressure problem, I take a pre and post, and there is often a significant difference in the two. Don't do it the 2 days before your physical, though: this might make you sore, and that wouldn't be good. Drink lots of water after your bodywork.

2 days before:

No weight lifting. As much as I push lifting weights, it does elevate some people's blood pressure. Usually it's very temporary, but better safe than sorry.

Day of:

Don't talk to your ex or anyone with whom you might have an upsetting conversation.

Try not to focus on any problems financially or otherwise that are due that you might be stressed about. During the exam, try to think of a relaxing thing: a quiet empty field or meadow, or sailing on a calm quiet summer day. Take long slow deep breaths.

Minimum Requirements:

The following statement represents the minimum training and experience standards which will be used to admit or reject applicants for tests. Applicants must meet requirements by time of appointment to the class of Firefighter.

EDUCATION:
Equivalent to the completion of the twelfth school grade.

RESIDENCY REQUIREMENT:
Philadelphia residency is not required at the time of application. Once hired, employees have six (6) months to establish bona fide residence in the City of Philadelphia.

ELIGIBLE LIST
OPEN-COMPETITIVE. Only an open competitive eligible list will be established as a result of this examination. Permanent City employees will receive no promotional preference in this examination. This list will remain in effect for four (4) years.

U.S. VETERAN'S PREFERENCE
Ten points for veteran's preference will be added to the scores of eligible veterans who apply for open competitive tests and pass the examination. You must be honorably discharged from the U.S. armed forces. Attach a copy of your DD214, Member 4, to your application if you wish your eligibility to be reviewed. Spouses of disabled or deceased veterans may also be considered.

LEGACY PREFERENCE
The children of Philadelphia Firefighters or Police Officers who were killed or died in the line of duty shall be accorded a preference in open competitive civil service entrance examinations. Candidates who qualify for this preference and who take and pass open competitive civil service examinations shall have ten points added to their final score. The candidate must successfully pass all parts of the examination before points are awarded.

Application Procedure:

As a condition of employment with the City, any person offered employment on or after the effective date of this regulation shall be required to certify that such person is fully current on any and all debts, taxes, fees, judgments, claims, and other accounts and obligations due and owing to the City; and shall be required to promise that such person will remain fully current throughout employment with the City. Any person who is or becomes delinquent on such obligations shall be required, as a condition of employment, to enter into a payment agreement. For so long as the person is employed by the City, the amount of the payment as set forth in the payment agreement (not to exceed twenty percent (20%) of the person's gross pay without the person's consent) shall be withheld from each paycheck until all debts, taxes, fees, judgments, and other accounts and obligations due and owing to the City are fully satisfied.

Education/Experience Requirements for Firefighter:

Applicants for the beginning position of firefighter must meet the following education/experience requirements.

* High School diploma/GED
* Texas Paramedic certificate, Texas license or National Registry Paramedic certificate. Applicants who posses a National Registry Paramedic certificate must apply for reciprocity with Texas Department of State Health Services before a start to work date can be issued.

Physical/Mental Health Requirements:

All applicants will be required to pass a pre-employment physical agility test to demonstrate their ability to perform the essential functions of the job as described in the official city job description.

All applicants will be required to pass, prior to appointment, a post-employment medical and psychological examination to determine the applicant's mental and physical ability to perform the essential functions of the job. Said examinations shall be paid for by the city. No applicant shall have a physical or mental health disability which would serve to cause an employee subject to these rules to be unable to adequately perform the essential functions of the job of firefighter or would otherwise tend to endanger other employees while performing such duties.

Drug Usage:

* No applicant may be certified as eligible for appointment to the position of firefighter who has used marijuana within the previous two years or who has sold or distributed marijuana within five years preceding the date of application.
* No applicant may be certified as eligible for appointment to the position of firefighter who has used, sold or distributed any other controlled substance without a prescription for a period of five years preceding the date of application.

All applicants shall be required to pass an Oral Review Board as established by the respective department heads and an in-depth background investigation including polygraph testing.

Cause for Directors Rejection of Application

* The applicant has failed to comply with any of the minimum requirements or qualifications for a beginning position as established by Chapter 143 or the Local Rules.
* The applicant has been convicted of a felony or any other crime involving moral turpitude.
* The applicant fails to pass the physical agility test established by the department to assess the ability of a firefighter to adequately perform the essential function of the job.
* The applicant is not of good moral character, temperate habits, sound health, or fails to possess sufficient temperament or ability to adequately perform the essential functions of the job as a firefighter as established by the following:
1. Background investigation into personal history and character reference
2. Polygraph examination
3. Medical examination
4. Drug test
5. Psychological test
6. Personal interview with the Department Oral Review Board and
7. Personal Interview with the department head

* The applicant has made a false statement in any material fact or practiced or attempted to practice any deception or fraud in the application process.
* The applicant is unable to intelligently read or write the English language.
* Applicants for beginning firefighter positions have no appeal rights except for physical and/or psychological examinations as provided for in Chapter 143, Section 143.22.
* Applicants who have failed the written or physical agility test are eligible to retest after six months. Applicants who fail the background investigation, polygraph test, psychological examination, the interview with the Oral Review Board or the Chief, or the drug screening for marijuana or alcohol are not eligible to retest for a period for two years. If the applicant fails the drug test due to the presence of other illegal substances other than marijuana or alcohol, then the retest period is five years.
